Medical Cranial Prosthesis (Lace Wigs) Insurance
Lace Wigs in the medical community are called Cranial Prostheses.
Divas Lace Wigs DOES NOT process insurance claims and we DO NOT accept prescriptions. However, we will assist you
through the entire process.
Before you do anything, contact your health insurance provider to find out if your policy covers cranial prostheses or lace
wigs and if so, the coverage amount. Also, find out exactly what they need from you to cover your purchase. Make sure you
follow all instructions given to you by your health insurance provider or your claim can be denied. Make sure you keep
copies of all of your documentation and invoices for tax purposes, if applicable.
Note: When speaking with your health insurance provider, you can use the term “Cranial Prosthesis or Lace Wig.” (they won't hold it against
you if you say "lace wigs." )
Based on our experience, many insurance companies will pay for or reimburse between 70-100% of the cost for a Cranial
Prosthesis. (The amount covered is based on the type of policy you have.) Generally, the health insurance provider usually
requires a prescription from your physician, oncologist or dermatologist for a cranial prothesis and a paid invoice.
Most insurance companies will require a prescription from your physician or oncologist for a cranial prosthesis and a paid
receipt.
Grants or Vouchers
If you don't have health insurance, contact the following organizations to determine if they will assist you with a Grant
or Voucher to purchase a cranial prosthesis.
It is also suggested that you obtain a letter of medical necessity from your physician, oncologist, or dermatologist.
This will be an explanation of the need for the medical cranial hair prosthesis and the medical cause for the loss of
hair.
Information necessary for grants or vouchers:
- A Prescription for a cranial hair prosthesis from your physician, oncologist or dematologist.
- A letter of medical necessity. This will be an explanation of the need for the cranial hair prosthesis, the reason
you cannot afford to purchase the cranial prosthesis on your own. and the medical cause for the loss of hair.
- An invoice for the cranial prosthesis. (Do not pay for the unit first or the grant or voucher may be denied.)
1. National Alopecia Areata Foundation (NAAF)
NAAF established the Ascot Fund, which is a special program that provides financial assistance toward the purchase
of a Cranial Prosthesis to individuals who have alopecia and are struggling financially. In order to request a grant,
you will need a diagnosis of Alopecia Areata from your physican, document of financial hardship, and an unpaid
invoice showing the cost of the prothesis.
You must apply first before purchasing your crainal prosthesis, as awards are not available post-purchase.

Upon receipt of your information, the committee will review your application and if deemed eligible, may award you up
to $500.00.
2. American Cancer Society (ACS)
The ACS provides free wigs and/or assistance for any cancer patent, whether insured or not. You MUST apply first
before purchasing your cranial prosthesis, as the voucher will not be awarded or even considered if you already
purchased the cranial prosthesis.
To inquire about getting financial assistance to purchase a cranial prosthesis or to find out if you qualify, contact:

If the American Cancer Society accepts your claim, they will issue you a voucher in the amount determined by the
American Cancer Society to help defray the cost of your lace wig.

If you have hair loss from any medical condition such as Cancer or Chemotherapy,
Alopecia, Trichotillomania, Lupus, psoriasis or other medical conditions that has caused
you to lose your hair, you may be eligible to receive a full or partial refund or financial
assistance from your health insurance provider to purchase a "Full Cranial Prosthesis." If
you don't have health care insurance, you might be eligible to receive a voucher or grant
from one of the organizations listed below.
